5 Easy Actions You Can Take To Complete Your California Phlebotomy Certification

It is required in the state of California for phlebotomy technicians to receive their phlebotomy certification. To apply for state certification, you must have documentation of completion of all required courses, clinical procedures (venipunctures), and pass a state-approved certification exam.

In California, you are encouraged to apply online through the California Department of Health – Laboratory Field Services website. No application will be processed without paying a $100 non-refundable application fee.

Now is the time to apply for your California-issued Phlebotomy Technician Certification and we’ll help you do it in 5 easy steps.

Action 1: Go to the California Department of Health: Laboratory Field Services and open an account with a username and password.

Action 2: Submit an electronic application online. You will pay a non-refundable application fee of ONE HUNDRED DOLLARS – (Visa or MasterCard).

Action 3: At the end of your application procedure, print the attestation page (a page attesting to the accuracy of all information entered) and print a duplicate “documentation of suggested experience form.”

Action 4: Compile all appropriate documents including: official transcripts, confirmation of training and experience, and certificate of completion of the training program, two passport-size photos, and state-sanctioned exam. You will submit these documents along with the attestation page in step 5

Action 5: Mail the Attestation page from action 3, plus all other forms required for phlebotomist credentials from step 4 to the LFS. LFS will not review an application if LFS has not received a certification page. You must have all correspondence addressed: “Warning: Phlebotomy Program”.
